Everything you need to know before we partner together

  • Our offices are located in the EPIC Mortgage building at 812 Cormier Road in Ashwaubenon. Please park in the right side lot and enter through the double doors also located on the right side of the building.

  • Our first visit typically involves getting to know your therapist, discussing what brings you to counseling, your goals, and beginning to build a supportive therapeutic relationship. It’s a safe, confidential space designed to help you explore challenges and start making progress.

  • We are considered out-of-network providers, which means we are not contracted with any insurance companies. However, some people have more out-of-network benefits than they realize when it comes to mental health!

    If you’re interested in using out-of-network benefits, we encourage you to call the number on the back of your insurance card and ask:

    • Does my plan offer out-of-network coverage for mental health services?
    • What is my annual deductible for mental health benefits?
    • What is my co-insurance (the percentage my plan will reimburse)?
    • Is there a limit to how many sessions are covered each year?

    It can also be helpful to ask how your insurance company prefers you to submit Super Bills. Cook Counseling & Therapy can email monthly Super Bills to all clients for their own reimbursement submissions.

    Our online system accepts any payment method that functions like a credit or debit card, including most HSA and FSA cards.

    We also welcome third party billing, especially when we are working with ecclesiastical organizations.

  • We understand that needs can change. If you find your current package isn’t the right fit, you have the option to switch to another therapist within Cook Counseling & Therapy. If you choose not to continue, we’re happy to provide reimbursement for any unused portion of your package.
